HOW TO PRE-ORDER FOR HELEN KELLER

Pre-orders are for Helen Keller Hospital staff only.

1. Please fill out and submit this form to place your pre-order.

2. After submitting, kindly scan the Helen Keller QR code and, on the payment page, pay the total for the meals you selected.

3. Once payment is made, your order will be confirmed.

We kindly ask that orders be placed by 5:00 PM the day before delivery.

We also recommend taking a screenshot of your order and payment confirmation to show us upon pickup if we need to verify any details. We will match the name on the credit card payment with the name you provided on this form under "name on credit card"
